Sonography is the mainstay of imaging the pleural cavity. Aspiration of the pleural fluid under ultrasound guidance has been used to aid in the diagnosis of specific diseases such as malignant, tuberculous, and empyematous pleuritis. To determine the accuracy of ultrasound-guided, fine-needle aspiration of the pleural fluid, 51 procedures in 50 patients were performed. The results of the cytologic interpretation were positive in 18 patients, negative in 11 patients, and indeterminate in 22 patients. The cytologic interpretation was positive in seven of seven patients with inflammatory cells in pleural fluid by flow cytometry. In the 22 patients with indeterminate cytologic results, follow-up information was available, and all of these patients had a definitive diagnosis of pleural disease on subsequent thoracotomy or thoracoscopy and/or follow-up. The results of ultrasound-guided, fine-needle aspiration of the pleural fluid correlated well with the final pathologic diagnoses in patients with malignant, tuberculous, or empyematous pleuritis. The interpretation of pleural fluid in patients with an indeterminate result or in those patients with a diagnosis of lymphoma on initial fine-needle aspiration cytology might be improved
